Registration
1. A single registration
form (Form A) is
to be filled for pre-conference satellite meetings,
CME, main conference and the workshop. Form may be photocopied, if required.
2. Registration
fee should be paid as per the rates below by a demand draft payable
to
'ISG 2001' at Lucknow. Cheques are not acceptable. Delegates will receive
a receipt for
payment made. They are requested to bring this receipt to the registration
counter;
registration is not guaranteed without the receipt.

3. Postgraduate
trainees must submit a certificate of their status from the head of
their
department or institution, with a valid stamp. Without such certificate,
discounted rates
will not be applicable.
4. Spot registration is limited and registration kits are not assured.
5. Registration will entitle the delegate to conference material, attendance
at scientific
meetings and conference meals.
6. Cancellation
of registration is permitted up to October 15, 2001. A processing fee
of
25% will be deducted and refunds will be made only after the conference
is over.
